The Putnam Food Service Department is conducting a brief survey on the free summer meal program. We have received many positive comments but are always looking to improve. Our goal is to solicit feedback from those families who use the program so that we can make it even better. We take serving healthy, nutritious meals very seriously and value your opinion. We would appreciate it if you would take time to complete the following brief 9-question survey.
Thank you so much for your help and participation.
