Athletic Forms

According to state law, the following forms are required in order to be eligible to participate in Staples Athletic Program:

1. Have a valid Physical Examination on file in the School Nurse's office (yearly).
2. Have a signed Sudden Cardiac Arrest Awareness Student and Parent Informed Consent Form submitted to the Athletic Director..
3. Have a signed Student and Parent Concussion Informed Consent Form submitted to the Athletic Director..
4. Have a signed Athletic Handbook Signature Page submitted to the Athletic Director.
